a. Fill Image with Pattern   

     1) On the First Tab of the Tool Option Palette:

           Blend Mode: = Normal
           Match Mode = None
           Tolerance: = Default
           Opacity = 100
           Sample Merged = Unchecked

           Ignore Second Tab of the Tool Option Palette

     2) Click anywhere on the new image

b. Apply Buttonize Effect

     1) Click Effects then 3D Effects then Buttonize...

           On the Buttonize Window:
           Height = 20
           Width = 20
           Opacity = 100
           Edge = Transparent Edge Checked
